ExemptTHE POSITION Washington County Behavioral Health Division within the Department of Health and Human Services is hiring for an is recruiting for a Oregon State Hospital (OSH) Care Coordinator to join our Behavioral Health Forensic program.
About the position:
This exciting position coordinates services for individuals with mental health needs who are criminally committed to the Oregon State Hospital.
Heavy emphasis is on strong collaboration across systems, including community mental health services, jail medical services, and the courts.
Primary duties may include, but are not limited to:
Conduct Community Restoration Consultation Evaluations; complete related paperwork and attend and testify at hearings as necessary.
Develop Community Restoration Plans based on identified needs; work with the patient, the court, attorneys, collaterals, and providers to implement and monitor the ongoing efficacy of the Plan.
Refer clients to outside agencies for social, economic, housing, and medical services as needed; arrange and coordinate service provision for those in and out of jail and OSH custody.
Provide mental health and Aid and Assist education services to clients, families, provider agencies, and other stakeholders.
Perform PSRB, Community Restoration, ICP and Jail Diversion back-up support to those teams as requested by the supervisor.
Attend advisory committees and coordinate meetings, as necessary.
Maintain documentation including written documentation of all contact with the client, natural supports, system, and service providers.
Collaborate with other Aid and Assist and ICP team members in connecting AIC's with appropriate services and supports.
This position is eligible for a hybrid work schedule including up to 5 days/week of remote work; however, the incumbent must reside within the state of Oregon or Washington and be able to report to Hillsboro, Oregon for an in-person meeting once a month and as needed.
This position also requires working at the Oregon State Hospital in Salem, Oregon, as needed.
In-person attendance may be expected on remote work days to support client care.
What we are looking for:
The ideal candidate will have an in-depth understanding of the intersection of criminal justice and mental health, and the Aid and Assist process.
They also have experience coordinating behavioral health services for people with mental health needs and have experience facilitating cross-systems problem solving.
Additionally, they will have experience advocating for justice involved individuals and working with community mental health and addictions services providers.
